Dental Protocol

The following information can be provided to your dentist.

Prophylactic antibiotic coverage for dental procedures is recommended ONLY for the following transplant recipients:
  • Prosthetic cardiac valve
  • Previous infective endocarditis
  • Congenital heart disease (CHD)
    • Unrepaired cyanotic CHD
    • 6 months following repair of CHD with any prosthetic material or device
    • Repaired CHD with residual defects
  • Cardiac transplant recipients who develop cardiac valvulopathy
Recommended prophylactic antibiotics to be given 30 to 60 min prior to procedure:

Amoxicillin                2 grams                        50 mg/kg

Ampicillin                              2 grams IV or IM         50 mg/kg IV or IM

OR

Cefazolin or Ceftriaxone     1 gram IV or IM           50 mg/kg IV or IM

OR

Clindamycin                                       600 mg                        20 mg/kg

OR

Azithromycin or Clarithromycin       500 mg                       15 mg/kg

Cefazolin or Ceftriaxone2     1 gm IV or IM           50 mg/kg IV or IM

OR

Clindamycin                           600 mg IV or IM       20 mg/kg IV or IM



 

1 Or other first or second generation oral cephalosporin in equivalent dosage.
2 Cephalosporins should not be used in an individual with a history of anaphylaxis, angioedema or urticaria with penicillin or ampicillin.
